Submitting Your Annual Wellness Form

Follow these steps to submit your form:

1. Log into your ActiveCare dashboard. Click the Incentives & Rewards tile

2. Click View My Incentives Programs

3. Click View Program within the Wellness Incentive Program

4. Click the Plus Icon within the Annual Wellness Exam Ribbon

5. Click Upload File
